SERVICE SETTINGS
Domain NamePlease provide the domain name of the website where you would like the script installed
FTP UsernameWhen you set up your account, your hosting company provided an FTP username
FTP PasswordWhen you set up your account, your hosting company provided an FTP password
Control Panel AddressPlease provide the web address you use to manage your account
Control Panel UsernameThis may be identical to your FTP username
Control Panel PasswordThis may match your FTP password
Install DirectoryIf you would like the script installed to a particular directory, please provide the directory name(optional)
Script NamePlease provide the name of the script you would like installed (and version number if you do not want the latest stable release)
